Zugan Health LLC (“Zugan”) is committed to protecting the privacy of the users of this website and our mobile applications.

This privacy policy has been compiled to better serve those who are concerned with how their personal information (including ‘Personally identifiable information’ (PII) and health information) is being used by Zugan. Please read this policy to understand how your personal information will be treated.

This website and the Zugan mobile applications have security measures in place to help protect against the loss, misuse, or alteration of information under our control. These measures include encryption of data using the Secure Socket Layer (SSL) system. Despite these measures, the confidentiality of any communication or material transmitted to or from us via this site or the Zugan mobile applications by Internet or email cannot be guaranteed.

We may from time to time amend this Privacy Policy. We post the effective date of our Privacy Policy on our website. If we make material changes to this policy we will post the revised policy and the revised effective date on this website. What Personally Identifiable Information is collected from you?
If you choose to sign up for one of our newsletters, inquire about our services, or decide to schedule an appointment with Zugan, we will ask for certain personal information, including but not limited to your name, email address, mailing address, phone number, credit card information, as well as certain health-related information.

How will Zugan use information collected from you?
Use and disclosure of health information includes using the information to provide treatment to the individual, to make payments for such treatment, and to conduct ongoing quality improvement activities. Our use and disclosure of an individual’s personal information (including health information) is limited as required by state and federal law.

Our uses of your personal information are outlined below:

1. Web Logs
We maintain standard web logs that record data about all visitors to our site. The web logs include but are not limited to the number of times a web page is visited, source referred the visitor to the site, the amount of time a visitor spends on a web page, and the type of browser and screen resolution to view a web page. This information is used to improve the user experience and customer service of our website.

2. Cookies
We may place Internet “cookies” on the computer hard drives of visitors to the Zugan website or mobile applications. Information we obtain from cookies helps us to tailor our site and mobile applications to optimize their user experience. The cookie consists of a unique identifier that does not contain information about you or your health history.

3. Surveys
We will on occasion ask users to complete surveys asking them to rate their experience with Zugan. Our survey asks our users to provide their demographic information such as age and to rank their experience with us against a number of pre-determined categories. We will never request users to disclose specific information about any medical information. The data collected in our surveys is used to improve the way we provide customer service to our patients and to measure the effectiveness of the programs we implement.

4. Credit Card Information
If you choose to schedule a visit with us, we will require you to provide us with information of your credit card for service billing purposes. We will treat your credit card number in a secure manner.

5. COPPA (Children Online Privacy Protection Act)
We do not specifically market to children under 13 and will not knowingly allow children under 13 to access secured features of the site.
